Acupoint catgut embedding is a method of treating diseases by embedding catgut into acupoints and utilizing the continuous stimulation effect of catgut on acupoints. It is commonly used for asthma, stomach pain, diarrhea, enuresis, facial paralysis, epilepsy, lower back and leg pain, flaccidity syndrome, as well as sequelae of poliomyelitis and neurosis.

Skin disinfection products, hole towels, syringes, forceps, thread embedding needles or modified No. 12 lumbar puncture needles (with the front end of the needle core ground flat), needle holders, No. 0-1 chrome catgut, 0.5-1% procaine hydrochloride, scissors, disinfectant gauze, and dressings. Buried needle is a tough and specially made metal crochet needle, approximately 12-15cm long, with a triangular tip and a notch at the bottom. If using the incision method, it is necessary to prepare a pointed surgical blade, surgical knife handle, triangular suture needle, etc.
Multiple acupoints should be selected for burying the thread in areas with relatively plump muscles, with the back, waist, and abdominal acupoints being the most commonly used. For example, for asthma, take Feishu, for stomach diseases, take Pishu, Weishu, Zhongwan, etc. The principle of selecting acupoints is the same as acupuncture therapy. But the selection of acupoints should be simplified, with 1-3 acupoints buried each time, and treatment can be performed every 2-4 weeks.

Method of embedding thread at acupoints
(1) Puncture needle embedding method: Routine disinfection of local skin, forceps take a section of sterilized catgut about 1-2cm long, place it at the front end of the lumbar puncture needle tube, connect the needle core, tighten or pinch the skin of the needle insertion site with the left thumb and index finger, hold the needle with the right hand, and puncture to the required depth; When a needle sensation occurs, push the needle core while retracting the needle tube, and implant the catgut into the subcutaneous tissue or muscle layer of the acupoint. Cover the needle hole with disinfectant gauze.
You can also use the needle tip of the 9th injection needle as a sleeve, and the needle tip of the 28th 2-inch long millineedle can be cut off as a needle core. Place the 00 # catgut thread 1-1.5cm into the needle and bury it in the acupoint. The operation method is as follows.
When using a specially designed embedding needle for thread embedding, after local skin disinfection, use 0.5-1% procaine hydrochloride as infiltration anesthesia, cut a section of catgut (usually about 1cm long), cover it on the notch of the embedding needle tip, and clamp both ends with vascular forceps. Hold the needle in the right hand and the forceps in the left hand, with the needle tip notch inserted downward at a direction of 15-40 degrees. When the needle tip notch enters the skin, the left hand will release the vascular clamp, and the right hand will continue to insert the needle until the intestinal thread head is completely buried under the skin. Then, insert the needle 0.5cm, and then withdraw the needle. Press the needle hole with a cotton ball or gauze for a moment, and then cover the wound with gauze to protect it.
(2) Triangular needle embedding method: Use gentian violet as a marker for the entry and exit points of the needle at a distance of 1-2cm on both sides of the acupoint. After skin disinfection, use 0.5-1% hydrochloric acid procaine for intradermal anesthesia at the marked area. Use a needle holder to clamp the skin suture needle with catgut thread, penetrate from a local anesthesia point on one side, pass through the subcutaneous tissue or muscle layer below the acupoint, and exit from the local anesthesia point on the opposite side. Pinch the skin between the two needle holes tightly against the skin, cut the ends of the thread, release the skin, and gently massage the local area to fully embed the catgut thread into the subcutaneous tissue. Apply gauze for 3-5 days.
1-3 acupoints can be used each time, and thread embedding is usually done once every 20-30 days.
(3) Incision and embedding method: Use 0.5% procaine hydrochloride as infiltration anesthesia on selected acupoints, use a knife tip to puncture the skin (0.5-1.0cm), first probe the vascular forceps deep into the acupoints, and massage the sensitive points through the superficial fascia to the muscle layer for a few seconds, then rest for 1-2 minutes. Then use 4-5 0.5 to 1.0cm long catgut threads to bury them in the muscle layer. The catgut should not be buried in the fat layer or too shallow to prevent absorption or infection. Suture the incision with silk thread, cover with disinfectant gauze, and remove the silk thread after 5-7 days.
Precautions for acupoint catgut embedding
(1) Strict aseptic operation to prevent infection. When embedding a triangular needle, the operation should be light and accurate to prevent needle breakage.
(2) It is best to bury the catgut between subcutaneous tissue and muscles, and it can be buried in the muscular layer in areas with abundant muscles. The catgut should not be exposed outside the skin.
(3) According to different parts, grasp the depth of the buried wire, and avoid damaging the internal organs, large blood vessels, and nerve trunks (do not directly ligate nerves and blood vessels) to avoid causing dysfunction and pain.
(4) When there is an infection or ulcer in the skin, it is not advisable to bury the thread. This method is not suitable for active pulmonary tuberculosis, bone tuberculosis, severe heart disease, or pregnancy.
(5) After using the leftover catgut, it can be soaked in 70% alcohol or treated with bromogeramine, and then soaked in physiological saline before use.
(6) When performing multiple treatments on a single acupoint, one should deviate from the previous treatment site.
(7) Pay attention to postoperative reactions and promptly handle any abnormalities.
